1. Whether you book your appointment directly through our office or via Zocdoc (Zocdoc new patient scroll below), please fill out the following new patient forms.
You can either bring the completed forms with you to your appointment or scan and email them back to our practice. This helps streamline your check-in process and ensures we have all the necessary information to provide you with the best care.

3. Please make sure to stop only your antihistamines a minimum of 2 to 5 days prior to your new patient appointment. Antihistamines include: Allegra (Fexofenadine), Zyrtec (Cetirizine), Xyzal (Levocetirizine), Claritin (Loratadine), Benadry (Diphenhydramine).

*Important Information for
Patients who booked through ZocDoc
1. Appointment Confirmation: After booking on ZocDoc, please call our office at 202-342-1984 or 703-425-8616 Monday - Friday 9:00AM-5:00PM to confirm your appointment in our system.
2. Health Insurance Verification: Your insurance must be verified before your appointment. If it cannot be verified or is not accepted by our practice, we do offer Out-of Network Insurance and Self-Pay Options (see #3).
3. Out-of-Network Insurance and Self-Pay Options: If we do not accept your insurance, we would still be happy to assist you with your medical needs as a self-pay patient. Please let us know if you would like to proceed under our self-pay option, and we can provide you with further details.
4. New Patient Forms: Print and/or fill out to complete the required forms before your visit. You can bring them with you or email them to us. Please arrive 10-15 minutes prior to your appointment time.

Office Policies Regarding Insurance Coverage
Confirmation of Eligibility
If you do not know your allergy/asthma health insurance plan benefit coverage, you should contact your insurance company for an explanation of your coverage benefits/deductible/copay, etc., according to your contract with them.
Filing a Claim
You should bring a copy of your health insurance card to evaluate your eligibility in order to provide you with an insurance verification form. We will be happy to file for you to facilitate payment.
Deductibles, Copays, and Patient Portion
All payments such as deductibles, copay, and the patient's portion should be made by the patient/parent at the time medical services are rendered.
If you do not have insurance, full payment is expected at the time of the visit.
Please understand that YOU are financially responsible for all services rendered.
We keep your account updated in our office regardless of your Insurance payments.
We are eager to help you receive the maximum reimbursement from your health insurance according to your contract and the services rendered.
